To update your surname on your HIF membership; simply complete the Federal Government Rebate form (so that we have your updated Medicare card information) and attach proof of your name change.
Supporting documentation:
- a copy of your marriage certificate
- a copy of your updated driver's license
- a copy of your divorce certificate
